Helen Mae Forman, 86, of Evans City died Thursday at Passavant Retirement Community in Zelienople.

Born Sept. 23, 1920, in Butler, she was a daughter of Hesper and Rose Wing- erter Edwards.

She was a homemaker. She worked for Mine Safety Appliance in Callery for 20 years until her retirement in 1982.

Her husband, Willy J. "Jim" Forman, preceded her in death on Aug. 13, 1975. Two sisters are deceased, and her daughter, Patricia Chine, also preceded her in death in 2006.

She was a Protestant.

She is survived by a daughter, Judy A. Forman of Evans City; two granddaughters, Mrs. Chris (Janis) Covert of Renfrew and Mrs. Todd (Rachael) Moore of Butler; and two great-grandsons, Hunter Moore of Butler and Cyle Covert of Renfrew. Numerous nieces and nephews, including Darwin "Punky" Edwards of Renfrew and Robert Collins of Butler, also survive her.

<B>FORMAN</B> — Friends of Helen Mae Forman, who died Thursday, April 19, 2007, will be received from 2 to 4 p.m. and 7 to 9 p.m. Sunday at <B>Geibel Funeral Home</B>, 201 E.
