Man, 18, accused of forging name on DUI consent form

BUTLER TWP — A suspected drunken driver is accused of forging another man's name on a PennDOT chemical testing form.

State police arrested Dalton J. Anthony, 18, of Oakland Township, on Tuesday. Anthony remains in Butler County Prison on $75,000 bond and faces a felony charge of forgery and misdemeanor charges of identify theft, false identification to law enforcement authorities, driving under the influence of a controlled substance and DUI by a minor.

Trooper Shaun Milkovich pulled Anthony over around 3:30 a.m. Dec. 21 for driving a car with a faulty brake light on East Metzger Avenue in Butler Township, according to court documents.

The officer smelled marijuana coming from the vehicle. During the police investigation, Anthony allegedly gave the name of a 19-year-old man.

Milkovich said the defendant also forged that man's signature on a PennDOT implied consent form, which police officers are required to read to motorists before requesting a chemical test for alcohol or drugs when DUI is suspected.

The warning puts motorists on notice that refusing breath or blood testing could lead to certain civil penalties, including license suspension for at least one year.

At Butler Memorial Hospital, a blood sample was drawn from Anthony for testing. Toxicology reports subsequently showed he had marijuana and alcohol in his system, police said.

It wasn't until later police determined Anthony's true identity.

District Judge Kevin O'Donnell arraigned the defendant and scheduled a Jan. 23 preliminary hearing. Online court records did not indicate if Anthony has an attorney.

The teen has been held in the prison since Tuesday following his arrest by Butler police on a bench warrant in an unrelated juvenile case.
