Charges added in prescription case

Zelie also accuses woman in scheme

A Chicago woman has received her fourth set of charges stemming from an alleged countywide prescription fraud scheme.

Zelienople Police on Friday charged Keeyonna R. Clark, 26, with multiple misdemeanors: two counts of forgery, one count of criminal attempt to procure drugs by forgery and one count of acquiring a controlled substance through misrepresentation.

The charges mirror three sets of charges brought against Clark over the past two weeks by Butler Township and Jackson Township police departments as well as state police.

According to charging documents, Zelienople's charges come from Clark's alleged attempt to fill a fraudulent codeine prescription at the Walgreens in Zelienople.

“The suspect was brought into custody by Butler Township (Police) and a bottle of codeine syrup purchased at Walgreens in Zelienople was collected,” said the investigating officer in the affidavit.

The documents said police identified Clark, who was wearing blue scrubs and a surgical mask, through collected video surveillance, which was also how she was identified in the other cases.

Police said they also later received a copy of the prescription that also matched those she used at other county pharmacies.

For the latest charges, Clark is scheduled to appear April 6 before District Judge Wayne Seibel. So far, two of the four cases have been held for the Butler County Court of Common Pleas.
