State police said they have determined the events that caused a crash Friday, May 26, in Concord Township which resulted in injuries to seven people, including three children.

First responders were called around 10 a.m. to the intersection of Hooker Road and Lou-Lin-Mar Lane, where they found a Mazda MX-3 overturned and a white Chrysler Pacifica with severe front-end damage.

According to police, the Mazda was carrying six people at the time of the collision.

Police said the Pacifica’s driver, Kaitlyn L. Leasure, 30, of Butler, was traveling east on Hooker Road and attempting to turn left onto Lou-Lin-Mar Lane as the Mazda was traveling west on Hooker Road.

The Mazda was driven by Jeffrey A. Wolfe, 40, of Karns City, and had five additional passengers.

Leasure turned her vehicle in front of the Mazda, and Wolfe attempted to swerve to the right to avoid a collision without success, police said.

The Pacifica turned 180 degrees on impact, coming to rest in the east travel lane of Hooker Road, police said. The Mazda hit an embankment on the right side of Hooker Road and overturned before coming to a stop, documents showed.

Wolfe was taken to AHN Grove City for suspected serious injury by Butler Ambulance Service.

A passenger in the Mazda, Rebecca L. Magini, 25, of Bruin, was taken by medical helicopter to UPMC Presbyterian Hospital in Pittsburgh for a suspected serious head injury, according to documents.

Also in the Mazda were two 3-year-olds and a 5-year-old, who each suffered suspected minor injuries. The crash report showed one 3-year-old was taken to AHN Grove City, and the other was taken to Butler Memorial Hospital.

According to Gage Ariss, assistant chief of West Sunbury Volunteer Fire Department, one child was taken from the scene to a hospital by a family member.

Police said Leasure was taken to AHN Grove City by Slippery Rock Volunteer Fire Company’s ambulance for suspected minor injuries.

Four ambulances were called to the scene, officials said.

Traffic was shut down on the intersection of Hooker and Hazlett roads while crews cleared the scene.
