New joint venture announced

CABOT — Officials at Concordia Lutheran Ministries announced a joint venture among Concordia Visiting Nurses, the Butler Health System, the Heritage Valley Health System, St. Clair Hospital and the Washington Health System.

This will promote a transition of care for patients from the hospital to their homes, according to a news release from Concordia Lutheran Ministries.

Martin Trettel, CEO of Concordia Visiting Nurses, said, “The partnership’s main focus is to provide quality care from the hospital to the home setting by offering skilled, wound and mental health nursing, physical, occupational and speech therapy, medical social services and home health aides.”

Trettel said the hospitals’ physicians and other clinicians will work with Concordia Visiting Nurses to provide care to patients.

Keith Frndak, CEO and president of Concordia Lutheran Ministries, said, “Our combined goal is to provide patient-centered care with a seamless transition from the hospital back into their home.”

Ken DeFurio, president and CEO of Butler Health System, said, “Strategic partnerships among successful, high quality providers are essential if independent organizations are going to continue to thrive in this ever-changing health care landscape.”
