John W. “Jack” Leitem, 84, of Slippery Rock and West Sunbury passed away early Friday morning, Dec. 14, 2018, at the VNA Inpatient Hospice in Butler.
Arrangements are under the direction of Steighner Funeral Home, 111 E. Slippery Rock St., Chicora.
