Amy Hortert of Butler was named director of care transformation at Concordia Community Support Services.

Hortert earned her undergraduate and master's degrees from Walden University, and her associate degree in nursing from Butler County Community College. She has extensive experience in hospice and home health care.

In this new position, Hortert will oversee executives as they adopt new strategies for the coordination and delivery of quality care.
