Quality Life Services is a family owned long-term care company that has been operating since 1973.

Beginning as Sugar Creek Rest in Worthington, Quality Life Services soon purchased Chicora Medical Center in Chicora, Trinity Living Center of Grove City and Fair Winds Manor of Sarver as well as four additional homes throughout Western Pennsylvania.

All Quality Life Services homes offer several services to best serve the community.

Memory Lane, a neighborhood dedicated to our residents who are living with dementia, is featured in the skilled nursing homes. Occupational, speech and physical therapy are all offered 7 days-a-week for our residents who are rehabilitating.

Sugar Creek Rest, Chicora Medical Center, Trinity Living Center and Fair Winds Manor all offer independent living, personal care and skilled nursing care on the same campus. This convenience allows residents to “age in place” by going from different care levels without leaving their neighborhood.

Sugar Creek Rest boasts a beautiful, country, 114-acre campus. Geese and ducks frequently visit the pond on the property while residents have the opportunity to feed them or go fishing for sunfish. Sugar Creek Rest is home to more than 100 elders. Meadow Lake Manor is the Personal Care neighborhood that is home to more than 40 residents.

The Chicora Medical Center is the only Eden Alternative home in Butler County. The Eden Alternative is a concept of culture change that has been adopted by all Quality Life Services Homes.

Instead of a traditional medical model of a nursing home, Quality Life Services focuses more on making their home a “home” for residents. More than 100 elders live at the Chicora Medical Center. Vista Royale is the Personal Care neighborhood that is home to more than 20 residents.

Fair Winds Manor has been a trusted community partner for more than 55 years. The lifestyle at Fair Winds Manor is one of freedom, elegance and dignity. It is home to more than 60 residents. The Personal Care neighborhood is home to more than 20 residents.

The Trinity Living Center is also an Eden Alternative home. More than 100 elders live in the skilled nursing neighborhood, and Personal Care is home to more than 20 residents.
