Phyllis M. Kozak, 91, of Cranberry Township passed away peacefully Saturday surrounded by her loved ones.

A strong-minded and compassionate person, Phyllis was beloved by many. She loved to play bingo, bird watch and spend time with her fur babies, Diamond, Tiny and Corey.

Her true passion in life was her family. There was nothing more important to her than spending time with her loved ones.

She leaves behind a legacy of strength, laughter and most importantly, love. Her memory will live on forever in the hearts of those who were fortunate enough to call her Mom, Grammy, Ball Ball, Aunt and friend.

She was the daughter of the late Andrew and Jean Kozak (Dunlap); sister of the late Frances Wenger (Kozak); former wife of the late Albert (Perle) Perlebach; cherished mother of Sonny, Jeannie (Jerry) Pratti, John (Irene) Perlebach and the late Joe Perlebach; and admired grandmother of Ed (Kari), Toot (Russ), Joey, Eric, Heather (Aaron) and Jessica.

She is also survived by many great-granddaughters, great-grandsons, nieces and nephews.

KOZAK — A celebration of life service for Phyllis M. Kozak, who died Saturday, Nov. 3, 2018, will be forthcoming. Details will follow via Facebook.Arrangements were handled by Boylan Funeral Home in Zelienople.In lieu of flowers, the family suggests memorial contributions be made in Phyllis Kozak's memory, c/o Boylan Funeral Home, 324 E.
