Utility contractor fined in fatality
Insight Pipe Contracting, an underground utility contractor headquartered in Lancaster Township, has been cited by the federal Occupational Safety and Health Administration after an employee was fatally electrocuted while on a job in Johnstown, Cambria County, earlier this year.
Christopher Diaz, 30, of Butler was leaning against a hydraulic forklift that came in contact with overhead power lines at about 10:30 a.m. on April 12 in Johnstown, according to police reports.
He was part of a crew that was working on a years-long project of the Johnstown Redevelopment Authority to replace the city's sewer lines.
Two other Insight Pipe employees were injured in the incident when they tried to assist Diaz. Both were hospitalized.
Department of Labor documents released Friday morning state that the three penalties issued to Insight Pipe total $217,285.
The three citations were:
The company did not document the completion of measures required to be taken before employees entered the 30-inch sewer pipe to adjust the pipe's liner
The employees who entered the pipe did not wear the required chest or body harness with a retrieval line
The inside of the pipe was not tested for oxygen content, flammable gases and vapors, or toxic air contaminants before employees entered, as required
The first citation was for $10,347 and the two others were $103,469 each.
Diane Reiber of Insight Pipe Contracting said the company's attorneys advised her not to comment on the incident or OSHA citations.
